Cancellation of appointments must be made 24 hours prior to the scheduled appointment. Late cancellations will be billed for the entire appointment fee. Health Insurance Companies do not cover cancelled appointments, so you as the client will be responsible for the appointment cancellation fee. As a Licensed Marriage & Family Therapist, I am able to submit claims for reimbursement to insurance companies directly. If you would like to utilize your Health Insurance benefits for therapy, please contact me with your insurance information; I am willing to assist you in determining your out-of-pocket cost for appointments. I do offer a reduced fee appointments during non-peak hours for individuals who are in financial need.
I am currently contracted as a Preferred Provider with Regence Blue Cross Blue Shield, Cigna, Aetna, Health Net,Optum, and MHN. I am also able to submit claims as an out-of-network provider to many other Health Insurance Companies. You, the client, are responsible for all fees that are not covered by your Health Insurance Company.
Payment is due in full at the time of each appointment. Acceptable forms of payment include cash and personal check. Credit card payments are also accepted using the Square Card Reader. A transaction fee of 2.75% is included with credit card payments.
