Professional Counseling for Individuals, Couples, & Families and Clinical Supervision
As a Licensed Marriage & Family Therapist, I am able to submit claims for reimbursement to insurance companies directly. If you would like to utilize your Health Insurance benefits for therapy, please contact me with your insurance information; I am willing to assist you and determine your out-of-pocket costs. I am currently contracted as a Preferred Provider with: Regence, Blue Cross Blue Shield, Cigna, Aetna, United Healthcare, Providence, Optum, Health Net, and MHN. I am also able to submit claims as an out-of-network provider to many other Health Insurance Companies. You, the client, are responsible for all fees that are not covered by your Health Insurance Company.
The fee for a 45-50 minute therapy session is $120. Payment and/or Co-payment is due in full at the time of each appointment. Acceptable forms of payment include: credit card, cash, and personal check. Upon request, I can create and send statements and receipts for services. At the termination of therapy, any outstanding balance will be billed to credit card on file to bring account to current.
Cancellation of appointments must be made 24 hours prior to the scheduled appointment. Late cancellations will be billed for the entire appointment fee. Health Insurance Companies do not cover cancelled appointments, so you as the client will be responsible for the appointment cancellation fee. Upon scheduling an initial appointment, you will be receiving an email with intake paperwork. Please take a few moments to review the forms and complete all requested information. This will ensure that our initial appointment will begin in a timely manner.
